Welcome to Orr Memory Clinic
Our clinic operates exclusively through a telehealth platform, allowing you to receive high-quality care from the comfort and privacy of your own home or nursing home. All appointments are conducted securely online, making it easier to access care without travel, waiting rooms, or added stress.
The nurse telemedicine call is for established clinic patients with prescheduled appointments only.
Coordination of Care Appointments
Our providers will gather more information about the patient to make sure we are doing all we can for good coordination of care for caregivers. They will be administering three standardized screening questionnaires and asking other questions primarily of the caregivers.

Before scheduling an appointment with us, please consult your insurance carrier. We are in network with Medicare and most major insurance plans. Patients are responsible for understanding their individual coverage, including deductibles and copays.
Once you have been scheduled, please follow the steps below to help us serve you better:
Step 1: Fill out clinic face sheet, notice of privacy practices, clinic authorization page, patient intake form and release of information by clicking the links below. Fill out forms completely, along with other pertinent medical information and send via postal mail, email (info@orrmemoryclinic.com) or fax (651-528-8184) to our clinic before scheduled appointment.
Step 2: Send in a current list of medications.
Step 3: Provide us with your insurance plan IDs, including your Medicare and/or Medicaid IDs.
Step 4: Bring a family member or caregiver with you to the appointment.

Please note our new No Show Policy: We will charge $75 for a missed appointment, or a cancellation of less than 24 hours prior to scheduled appointment.
